Curain Healthcare Services LLC, doing business as SYNERGY HomeCare of Clifton-Secaucus ("we," "us," "our"), provides home care and related services in northeastern New Jersey from our office at 2 Broad St, Suite 609, Bloomfield, NJ 07003. This Privacy Policy describes how we collect, use, and protect personal information through our website (www.curain.org) and in the course of providing our services.
Information We Collect
- Website contact form: name, email address, phone number, subject, and message, which are delivered to our office email (info@curain.org) so we can respond to your inquiry.
- Service intake: when you engage our services, we collect contact, scheduling, and care-related information through our service agreement and intake process.
- Employment: when you join our team, we collect contact and employment information through the employment agreement and onboarding documents.
- Communication records: the date, time, and content of text messages, and the date, time, and duration of phone calls with our office.
We do not purchase, rent, or scrape contact lists. We only contact people who have provided their information directly to us.
How We Use Information
We use personal information to respond to inquiries, coordinate and deliver home care services, schedule and manage caregiver shifts, meet our legal and regulatory obligations as a home care provider, and communicate with clients and caregivers as described below.
Text Messaging (SMS)
SYNERGY HomeCare of Clifton-Secaucus sends text messages for service coordination only:
- follow-up messages to clients when a caregiver was unable to reach them by phone before a first visit;
- missed clock-in or clock-out notifications to caregivers;
- upcoming-shift confirmation requests to caregivers.
We do not send marketing or promotional text messages. Clients consent to receive these messages at intake through our electronically signed service agreement; caregivers consent through the electronically signed employment agreement and onboarding documents. Each electronic signature is captured with a timestamped audit trail, which we retain as our record of consent. Consent to receive text messages is not a condition of receiving services.
Message frequency varies depending on your care or work schedule. Message and data rates may apply. You can opt out at any time by replying STOP to any message; you will receive one final confirmation message and no further texts. For help, reply HELP to any message, call our office at (862) 401-2233, or email info@curain.org.
Text messages are sent and received on our behalf by our communications service provider, Neovexa Inc, using the Twilio messaging platform. Both process phone numbers and message content solely to deliver our messages and never for their own purposes.
Telephone Calls and Call Recording
We may also contact clients and caregivers by telephone, including automated calls with a recorded or synthesized voice, for the same care-coordination purposes described above. You may opt out of automated calls at any time by contacting the office at (862) 401-2233 or info@curain.org.
Telephone calls with our office may be recorded for quality assurance and care-coordination documentation, with prior written consent or notice collected electronically with our intake and employment documents. For clients, recording is optional and consent may be withdrawn at any time without affecting your services. Recordings are stored on access-controlled systems, are used only for the purposes above, are never shared with third parties for marketing purposes, and are retained only as long as needed for those purposes and our legal obligations.
Mobile Information and Sharing
No mobile phone numbers or text-messaging opt-in data and consent will be shared with or sold to third parties or affiliates for marketing or promotional purposes. We share personal information only with the service providers who transmit messages and calls on our behalf (Neovexa Inc and the Twilio platform, solely to deliver communications), when required by law, regulation, or valid legal process, or with your consent or at your direction. Text-messaging originator opt-in data and consent are never shared with any third party for any other purpose. We do not sell your personal information.
Data Retention and Security
We retain personal information only as long as needed for the purposes above and to meet legal, regulatory, and accounting requirements. Information is stored on access-controlled systems limited to personnel and service providers with a business need, protected by administrative, technical, and physical safeguards appropriate to the sensitivity of the information.
Your Choices and Rights
You may request access to, correction of, or deletion of your contact information, and you may withdraw texting consent at any time (reply STOP or contact the office). New Jersey residents may have additional rights under applicable state law.
If you receive services from us, our Notice of Privacy Practices (provided at intake) describes how health information is protected. This page covers our website and communications.
Changes to This Policy
We may update this Privacy Policy from time to time. The current version will always be available at this URL, with the "Last updated" date reflecting the most recent revision.
